End User License Agreement
This EULA governs your use of the Blaylar Health Provider Portal, Patient Portal, and associated software applications.
1. License Grant
Subject to your compliance with this agreement, Blaylar Health grants you a limited, non-exclusive, non-transferable, revocable license to access and use the software for patient, provider, clinical staff, and authorized administrative workflows.
2. Restrictions
- You may not reverse engineer, copy, distribute, sublicense, or misuse the software.
- You may not use automated tools to access the software without written permission.
- You may not attempt unauthorized access to accounts, systems, or protected data.
3. Intellectual Property
The software, source code, interfaces, graphics, logos, documentation, and underlying technology are the property of Blaylar Health or its licensors.
4. Protected Health Information and HIPAA
- PHI is encrypted in transit and at rest.
- Access is controlled through authentication, authorization, and audit logging.
- Third-party vendors with PHI access must operate under appropriate HIPAA safeguards and BAAs.
5. AI-Powered Features
- AI-generated content is decision-support only.
- Clinical AI output must be reviewed and approved by a licensed provider before becoming part of the medical record.
- Patient-facing AI responses are not reviewed by clinical staff before display.
6. Telehealth, Prescribing, and Communications
Providers remain responsible for all clinical, prescribing, telehealth, fax, and communications decisions made using the platform, including compliance with applicable licensing and regulatory requirements.

Blaylar Health is not liable for indirect, incidental, special, consequential, or punitive damages arising from use of the software to the maximum extent permitted by law.
